The death of a 46 year old male construction foreman who fell 16 feet from the bucket of an aerial lift truck was reported. The victim entered the aerial bucket and did not don his safety equipment. A tractor trailer attempted to drive under the outstretched cables, catching the cables and knocking the victim out of the bucket, to the roadway. He died of internal hemorrhage the following day. Recommendations were offered to ensure that appropriate fall protection equipment is used when working where there is a danger of falling, that existing safety programs to be reviewed and that workers should be encouraged to actively participate in safety at the workplace.
