This case study highlights the various problems encountered in the operation of our phase II cooling tower fan and its structure. There was repeated failure of equipment's leading to extensive downtime. The reliability of the equipment was very low. A proactive (root cause based) approach was adopted to investigate the root cause for high vibration and frequent machinery failure. This new approach was very essential to maximize the life of the equipment and to keep the maintenance cost low, through systematic elimination of the causes of failure. Even though major replacement is envisaged, in the long run it will be very beneficial. The problem of bearing rubs, high crest factor & the problem on Expander - Compressor highlight the importance of time wave form analysis. All the case studies discussed in this article are based on our operational experience.
